If your PTA does not already have an employer ID number (EIN), you can apply for one online with the Internal Revenue Service at www.irs.gov.
Your PTA’s most recent meeting minutes must be provided at account opening as well as anytime modifications are made to the account. The minutes must state the names of PTA members who are to be listed on the account, including those authorized to open and make modifications to the account.
Each individual added to the account must provide their ID and Social Security number (SSN). Acceptable forms of identification are unexpired state or federal photo IDs including, but not limited to, driver’s license, state ID card, passport, and weapons permits.
Each person added to the account must sign account paperwork either at a Cyprus branch OR electronically via DocuSign.
A minimum deposit of $5 is required to open a savings account. If opening a checking account, an additional deposit of $50 is required into the checking account.
